§ 24-274. Permit application form.

Applicants shall submit the following information on an application form to be provided by the village:

(1)

The name, addresses, and telephone numbers of the applicant, contact persons, and individuals responsible for the day to day operations of the film production;

(2)

A description and summary of the type of film to be produced;

(3)

The proposed dates and hours for film production;

(4)

The proposed locations for film production, including staging and storage locations;

(5)

Identification of all public rights-of-way and public property for which the applicant seeks to use, requests closures or partial closures, and, for which the applicant seeks alterations of normal parking, vehicular, or pedestrian traffic patterns;

(6)

A description of the methods and procedures to be used for food service, emergency medical treatment, health and sanitation, and removal of garbage and waste; and

(7)

Such other and further information as the village may deem necessary to evaluate the permit application.
